Hello! My name is Sarah and today is my first day here at QC. I have a silly question to ask, where can I find the model feedback forms? I have the consent one, but not the feedback. Thank you in advance!!

Hello Sarah, I'm Megan and I've been here since about June...and still on Unit B. Taking my time to practice right? I was able to find it under the Unit B drop down and under handouts.
